Dear Prospective Participant,
My name is Lanna Lee Maheux-Quinn. I am a Sociology and Mathematics student from the University of Southern Maine. I am conducting an anonymous survey about happiness and subjective well-being. To participate, you must be an adult 18 years or older.
The survey is voluntary. You may quit taking this survey at any time.
Since your answers are to remain anonymous, PLEASE DO NOT PUT YOUR NAME ON THIS SURVEY.
The survey will take 5-10 minutes to complete. Please answer the questions to your comfort level.
The results will be reported for the group of respondents as a whole. Open-end responses may be quoted using the following demographic information as identifiers: gender, age and race. We will not include any information that will make it possible to identify a participant.
There are no known benefits for participating in this survey. Additionally, there are no known risks for participating in this survey.
